I have been told by
several people that it is not a good idea to allow access to the form by
allowing it to be filled in electronically on the web site. So It is OK
to place this form, in PDF format, just as it is. The intention is this
form will simply be printed out by the home owner and filled out by hand
and signed. The form should be given to the Architectural Control
Committee Chairperson or mailed to P. O. Box 992 Lake Dallas, TX. 75065.
The Architectural Control Committee will meet to consider the
request, make the appropriate decision, sign the form and then return
the form to the homeowner.
